🏥 The Hancock County Health Department will be on-site at Greenfield-Central’s centralized registration on Tuesday, July 21st, from 3-7pm. The vaccine clinic is open to the public. You do not need to be a GC student to attend. Immunization requirements in Indiana state that a student must meet one of three criteria:
1. Have a complete vaccination record, having received all of the required vaccines for the current school year. You can find a reference to those requirements here: https://www.in.gov/health/audiences/2026-27%20Required%20and%20Recommended%20School%20Immunizations.pdf
2. Annually file a medical exemption form if the student is unable to receive vaccines for a specific health reason. Indiana’s state-issued medical exemption form can be found here: https://www.in.gov/doe/files/medical-exemption-form-providers.pdf
3. Annually file a religious exemption. For Greenfield-Central, this can be an email or a handwritten letter. It must be submitted by a parent or guardian, and must include your student’s name, date of birth, and state that you religiously object to vaccines. You can state either “all vaccines” or list specific vaccines to which you object. The email can be sent to [email protected].
Providing a complete vaccination record, or an updated medical or religious exemption, before the start of the school year is helpful to school personnel responsible for verifying completion. Thank you for your attention to this matter.
