IMMUNIZATION REQUIREMENTS

GREENFIELD-CENTRAL COMMUNITY
SCHOOLS
NEW REQUIREMENTS FOR GRADES 6 - 12
For the
2010-2011 school year, students in Grades 6 through
12 will need to have had the following:
1.
Two
doses of chickenpox vaccine (Varivax)
on or after the 1st birthday OR have a
history of the disease
2.
One
dose of tetanus and pertussis (whooping cough) vaccine (Tdap)
on or after the 10th birthday
3.
One
dose of meningitis vaccine (Menactra
or
MCV4)
If your student needs any or all of
the above vaccines, please make an appointment as soon
as possible with your family physician or with the
Hancock County Health Department (477-1125).
Because these
are required for school entry, you will need to provide the school
corporation with your student’s updated immunization
record before the start of the 2010-2011 school year.
You may mail or bring it to
the School Administration Office this summer; the
address is 110 W. North St., Greenfield, Indiana 46140.
REQUIREMENTS FOR GRADES K - 5
Students
entering grades 1 through 5 have no new immunization
requirements for the 2010-2011 school year.
Students entering
Kindergarten have immunization requirements indicated on
the “yellow card” (provided at Kindergarten Round-Up);
this yellow medical record card is to be completed by
the student’s physician and returned to the School
Administration Office by August 1.
